In brief

A recent incident report has revealed that an unsecured penguin caused a helicopter crash shortly after take-off in South Africa. The crash occurred on January 19 when a specialist placed a cardboard box containing the penguin on his lap, which resulted in a dangerous situation when the box slid off at about 15 meters above ground.

The South African Civil Aviation Authority's investigation indicated that the lack of proper containment for the penguin led to the pilot losing control. After conducting an aerial survey of Bird Island, the helicopter was preparing to transport the penguin back to Port Elizabeth when the flight went awry. Although the pilot had conducted a risk assessment, transporting the penguin was not included in the evaluation, and the containment measures did not comply with safety regulations. As the helicopter was ascending, the box containing the penguin fell and knocked the cyclic pitch control lever, causing an extreme roll that made recovery impossible. The helicopter ultimately crashed on its side approximately 20 meters from the lift-off point, incurring significant damage but fortunately leaving all occupants, including the penguin, unharmed. The report emphasized the need for adherence to aviation safety procedures, adequate risk assessments, and established containment methods to avoid such incidents in the future.
