For the first time, a genetically‑modified pig kidney worked inside a human body for 271 days – the longest record for a xenotransplant. In January 2025, 66‑year‑old Tim Andrews, a Texas‑born former veterinarian with diabetic kidney failure, received the kidney at Massachusetts General Brigham following a surge of hope that such procedures could bridge the critical organ shortage that plagues many indigenous communities.
The pig used – a Yucatan miniature with 69 genomic edits – had human genes added to camouflage it from the immune system, and viral elements were disabled. Doctors noted that while the organ began to fail after six months, the body‑rejecting reaction had been mitigated, a success that shows both promise and the complexity of performing transplants across species.
For many indigenous peoples, the intersection of advanced science and animal stewardship is fraught with ethical concerns. Traditional teachings often view livestock as stewardship partners rather than commodities, and the use of animals for human medical benefit raises questions about consent, animal rights, and the potential for disruption of historically balanced ecosystems. Recent protests in the U.S. and Canada over livestock slaughter for research highlight the tension between biotechnological progress and indigenous values of reciprocity and respectful use of living beings.
The transplant offers a tangible lifeline for patients lacking human donors, reducing dependence on dialysis which is a common loss of life expectancy in underserved and indigenous populations. Yet the case also signals the need for policies that ensure equitable access, transparent consent, and community oversight: not merely a medical advancement, but a societal decision that affects seats at the table for all stakeholders.
Beyond the clinical details, the image of Andrews and his wife in a hospital corridor (figure 1) and the physician team in the operating room (figure 2) remind us that science is a shared narrative. The story invites indigenous communities to be partners in the future of xenotransplantation, shaping regulations that honor both human health and the wise stewardship of animals that has guided generations.
